Home Based Support Services

The Home Based Support Services Program is funded by the Illinois Department of Human Services. The program offers grant funds with which consumers can purchase services that will allow them to maintain home living. An external agency, Community Alternatives Unlimited (773-867-4000), manages the application process and submits completed applications to the Illinois Department of Human Services for approval. All consumers that are eligible for the Home Based Support Services Program must have a Service Facilitator. Resources for Living, Inc. accepts new Service Facilitation consumers throughout the year.

Once enrolled in the Home Based Support Services Program, we provide the following services:

  • Information and referral.
  • Attainment of adaptive equipment including home and vehicle modications.
  • Linkage to needed services.
  • Advocacy and follow-up.

Resources for Living, Inc. also provides Service Facilitation for the Home Based Support Services Program for the Mentally Ill if the consumer was accepted to our program before 2003. We are not, however, taking new clients for this program.
